The 2024 Affordable Car Act (ACA) 1095 Forms are now available by request.

• The 1095 forms will not be mailed as in past years.
• Individuals who wish to receive a copy of the 1095 can email Trisha Bourne at tbourne@westmiddlesex.org to have a copy emailed to you.
